I'd like to see visual changes to npc groups based on your standing with them and quests completed/failed/denied. My idea is for groups you help, you see them improve their compound. Clean it up, repair items, broken glass, etc and make it look welcoming. On the opposite side of the spectrum for groups you ignore or make hostile maybe the board up their windows, add spikey poles, or barbed wire to their fences. Take on a menacing look. Furthermore if a group asks for a generator or a bag of supplies maybe have that become a part of their scenery. Mechanics asking for fuel? Spread out a number of fuel can on the floor after you delivered. Doctors need a generator? Next time you visit once completed they have lights strung up with a power cord going to the generator you delivered. Make player actions to npc groups feel authentic like you actively helped or hindered them.
