So. I like Nightmare difficulty for its challenege but what if we made it even harder? So Plague Walls. Plague EVERYTHING. Zombies, Juggernauts, Ferals, Screamers, Bloaters. But get this. You have the Option for Nightmare Zone. Or Plague Zone. (They relate to the topic at hand) Just food for thought.
