Add new difficulty that adds Blood boater feral and screams from heartland

Not sure if it would be a difficult thing to do in adding content. Adding a new difficulty with all the blood pledge cast bloater screamers and ferals. But if the new mode can’t be made or difficult to change just add them to nightmare
